Getting Started With the Care Plan…

Once we are notified of your needs for home care an evaluation/assessment of the patient is conducted and a comprehensive plan of care is developed. This plan includes medical necessities as well as other elements all designed for patient improvement, safety and comfort. RN Case Managers who are in constant touch with the patient, caregivers and the family, supervise your case. Daily nursing notes are maintained and made available to the appropriate individuals.

It’s important to know that Firstat has Registered Nurse supervisors on duty and available 24 hours a day…even on the weekends and holidays. We never use answering machines or answering services…our own staff coordinators answer the phones 24-7. You are never out of touch, nor are we.
